Employee contributions to health insurance premiums including medical and dental are
automatically taken pre-tax unless you specifically decline this tax benefit.
Flexible spending options including pre-tax withholding of dependent care and
un-reimbursed medical expenses are available upon your request.
Deductions are taken from the bi-weekly payroll; reimbursement checks are generated weekly
following employee submission of receipts from eligible care providers. You
have up to 90 days after the end of the plan year, or termination of your
employment, to receive reimbursement of eligible expenses incurred during the
plan year.
