Select From Anthem Blue Cross HMO Or PPO
Flat rate of $135.00 per month contribution of employee-only premium is paid by Zebra-net;
Balance of employee-only + additional costs for spouse and/or children is paid
by employee through payroll deduction. Employee-paid premiums will be paid
pre-tax, unless you specifically decline this tax benefit (see Section 125).
Employees are given a selection of HMO and PPO plans to choose from.
Coverage is effective on the first day of the next month following hire date plus 30 days.
For instance, a hire date of 1.15.2008 will allow an insurance effective date
of 3.1.2008. Coverage will stay in effect until the end of the last month of
employment.
