There Are 5 Different Plans To Choose From

Flat rate of $15.00 per month contribution of employee-only premium is paid by Zebra-net; Balance of employee-only + additional costs for spouse and/or children is paid by employee through payroll deduction. Employee-paid premiums will be paid pre-tax unless you specifically decline this tax benefit (see Section 125).

Coverage is effective on the first day of the next month following hire date plus 30 days.
